Home health care is most often ordered by the physician following discharge from the hospital, or it may be ordered in lieu of hospitalization. In most instances it is not expected to be long term, but rather to help the patient and/or caregivers become independent in the necessary care.

Home health nursing is skilled nursing services provided by a registered or licensed practical nurse to persons in their home.

Services Provided Include:

Home Health Aide: to assist with hygiene care and to teach the patient and/or caregivers how to safely bathe and groom the patient. Home Health Aide also will obtain vital signs, look for abnormalities in skin turgor and report any abnormalities to a skilled nurse.
Nursing Care: complicated wound care, IV therapy (including PICC Line Care & Removal), blood testing, staple/ suture removal, injections, prefill insulin syringes, diabetes education, medication instruction, ostomy & gastrotomy instructions & care and tracheostomy instructions & care. Our nursing staff provide many more skilled nursing services as ordered by your physician.
Physical Therapy: for physical rehabilitation.
Speech Therapy: for speech rehabilitation.
Occupational Therapy: to reteach basic living skills.
Eligibility and Referrals:

Any person in Franklin, Williamson, Jackson or Johnson Counties who is home-bound due to a medical condition is eligible. Client referrals are accepted from physicians, family, friends, hospitals and social services agencies, provided that a physician certifies that a service is needed. Referrals can be made at any time.

Length of Service and Number of Visits:

The length of care depends upon the individual’s medical condition and the type of care ordered by the physician. Services may be provided for two months up to indefinite periods of time. The number of home visits made depends on the medical condition of the patient and the type of services needed. Visits may range from daily to weekly to monthly.

Costs:

We accept most forms of  private insurance, Illinois Public Aid Medical Card and Private Pay for services. . The Home Health Nursing program is certified by Medicare and is licensed by the State of Illinois.
